Assessments & Intake Tools

An assessment families finish — and act on

Intake forms collect data. A clinical assessment tells a family something they didn't already know about their own situation — and that's what makes them stay.

Why assessments beat forms

Most care platforms collect intake data: hours needed, budget, location, start date. Useful for matching a schedule, useless for matching a person. The family who leaves in month two usually filled out that form perfectly.

A real assessment reads the whole situation — biological, emotional, social, environmental — and produces a recommendation the family recognizes as true. That recognition is what converts a browser into a customer and a customer into a retained one.

Where organizations use them

How it's built

Licensing typically starts with one assessment for one audience — a pilot you can measure. From there it extends across tracks and surfaces. Implementation is collaborative: your team builds it into the product, I supply the clinical structure and review what ships.
