Online therapy for depression and feeling flat

Maybe it isn’t sadness, exactly. It’s flatness. Things you used to enjoy feel like chores, mornings are heavy, and you’re getting through the day rather than living it.

I’m Tammy Gold, a licensed clinical social worker. In therapy we look at what’s weighing on you, rebuild small sources of energy and meaning, and keep an eye on your safety along the way.

Online sessions for adults located in California, New Jersey, Oregon or Maine. Alma checks your specific plan before you book.

Who this is for

This page is for adults who have felt down, flat or hopeless for weeks, whether something specific set it off or it seemed to arrive on its own.

You might notice that:

This is individual psychotherapy with a licensed clinician. It isn’t coaching. Couples and family sessions are a different format that insurance may treat differently, so ask before you book if you’re interested in a joint session. Medication is a separate question: your primary care doctor or a psychiatrist can assess whether it could help, and with your permission I can coordinate with them.

If you need help now: If you’re thinking about suicide or harming yourself, call or text 988 now, or call 911. You don’t need to wait for a session.

What we might work on

How the Care Map Method™ shapes our sessions

The Care Map Method™ is the framework I use to organize our work, alongside a standard clinical assessment. It isn’t a separate tested treatment. It keeps four kinds of needs in view:

BIOLOGICAL

Sleep, appetite, energy, health and medication questions.

EMOTIONAL

Sadness, numbness, guilt and the story you tell about yourself.

SOCIAL

Who you’ve pulled away from, and who you can lean on.

ENVIRONMENTAL

Routine, work, money, home and daylight hours.

Then we work in four steps:

  1. Map the emotion and trigger. We chart a typical week: when mood dips, when it lifts even a little, and what came just before.
  2. Match the need with a strategy. We match one need, such as rest, connection or a sense of accomplishment, with one small action that fits your energy right now.
  3. Merge the strategy into daily life. We schedule it, make it small enough to happen on a bad day, and plan what you’ll do if it doesn’t.
  4. Manage communication and responses. We practice how to tell the people close to you what you need, then review the week together. Small, steady changes add up.
TRY THIS

The two-list evening

Before bed, write two short lists. First, three things you did today, however small: showered, answered one email, walked to the mailbox. Second, one thing you’ll do tomorrow that brings even a little pleasure or connection. Depression tends to erase what you did and magnify what you didn’t. The lists push back.

How we’ll look at progress

We’ll choose markers that fit you, such as:

We check these every few sessions and change the plan if something isn’t helping. I can’t promise a particular result; we focus on what is in your hands.

Sessions, licenses and insurance

Sessions are online by video for adults who are physically located in California, New Jersey, Oregon or Maine during each session. Licenses: California LCSW #136364; New Jersey LCSW #44SC05528100; Oregon LCSW #L16530; Maine LCSW #LC26593.

Through Alma, I’m in network with Aetna, Cigna, Carelon, Blue Cross Blue Shield of Massachusetts, and Optum plans including UnitedHealthcare, Oscar and Oxford. Coverage depends on your specific plan, not only the insurance company’s name. Alma checks it before your first session, including any copay or deductible, and handles the billing. Questions people ask

What’s the difference between depression and a rough patch?

Everyone has low stretches. When low mood, loss of interest, sleep changes or hopelessness last most of the day, nearly every day, for two weeks or more, or get in the way of work and relationships, it’s worth a professional assessment. I can do that in our first session.

Do I need medication too?

Some people do well with therapy alone; others benefit from both. I’m not a prescriber, but I can talk it through with you and, with your permission, coordinate with your doctor or a psychiatrist.

What if I can’t find the energy to start?

That’s common with depression. The first session asks very little of you: show up by video, from wherever you are. We start with what’s manageable.

What if I have thoughts of suicide while we’re working together?

Tell me. We’ll talk about it openly and make a safety plan together. Between sessions, call or text 988 any time, or call 911 in an emergency.

Do you take my insurance?

I see clients through Alma, and through Alma I’m in network with Aetna, Cigna, Carelon, Blue Cross Blue Shield of Massachusetts, and Optum plans including UnitedHealthcare, Oscar and Oxford. Coverage still depends on your specific plan. Alma checks it before your first appointment, including any copay or deductible. Please share plan and health details through Alma’s secure intake, not by text or email.

Where do I need to be during sessions?

Sessions are online by video, and you need to be physically in California, New Jersey, Oregon or Maine during each one. Your insurance plan can come from another state; what matters for licensing is where you are during the session. Alma checks whether your plan covers sessions with me.
