Online therapy for going back to work after baby

Your leave is ending, or it ended and nothing feels settled. You’re worried about the childcare, the pumping schedule, the meeting you’ll miss for pickup, and whether you’ll be good at any of it.

I’m Tammy Gold, a licensed clinical social worker. I’ve worked directly with families for more than 20 years, including on the childcare decisions behind going back to work. In therapy we work on the anxiety, the guilt and the plan together.

Online sessions for adults located in California, New Jersey, Oregon or Maine. Alma checks your specific plan before you book.

Who this is for

This page is for mothers and fathers returning to work after birth or adoption, whether the return is weeks away or already happened.

You might notice that:

This is individual psychotherapy with a licensed clinician. It isn’t coaching. Couples and family sessions are a different format that insurance may treat differently, so ask before you book if you’re interested in a joint session. Hands-on help choosing and hiring childcare is Care Map coaching, a separate service that isn’t billed to insurance.

How the Care Map Method™ shapes our sessions

The Care Map Method™ is the framework I use to organize our work, alongside a standard clinical assessment. It isn’t a separate tested treatment. It keeps four kinds of needs in view:

BIOLOGICAL

Sleep, feeding, pumping and recovery.

EMOTIONAL

Guilt, worry, grief, relief and ambition.

SOCIAL

Your partner, manager, coworkers, caregiver and family.

ENVIRONMENTAL

Commute, schedule, childcare, money and the house.

Then we work in four steps:

  1. Map the emotion and trigger. We walk through your first week back hour by hour and mark the moments you’re dreading.
  2. Match the need with a strategy. We name the need behind each one, such as reassurance, time or backup, and pick a strategy for it.
  3. Merge the strategy into daily life. We set the plan: who does drop-off, what the backup is, when you’ll pump, what you’ll protect.
  4. Manage communication and responses. We rehearse the conversations with your manager, partner or caregiver, then review what worked once you’re back.
TRY THIS

Do a practice run

A week before you go back, do a practice day. Get ready, do the full drop-off, then spend your working hours away, even at a café. Pick the baby up at the real time. You’ll find the snags early, and your baby and caregiver get a rehearsal too.

How we’ll look at progress

We’ll choose markers that fit you, such as:

We check these every few sessions and change the plan if something isn’t helping. I can’t promise a particular result; we focus on what is in your hands.

Sessions, licenses and insurance

Sessions are online by video for adults who are physically located in California, New Jersey, Oregon or Maine during each session. Licenses: California LCSW #136364; New Jersey LCSW #44SC05528100; Oregon LCSW #L16530; Maine LCSW #LC26593.

Through Alma, I’m in network with Aetna, Cigna, Carelon, Blue Cross Blue Shield of Massachusetts, and Optum plans including UnitedHealthcare, Oscar and Oxford. Coverage depends on your specific plan, not only the insurance company’s name. Alma checks it before your first session, including any copay or deductible, and handles the billing. Columbia University–trained therapist · University of Pennsylvania graduate

I’m a licensed clinical social worker (California LCSW #136364; New Jersey LCSW #44SC05528100; Oregon LCSW #L16530; Maine LCSW #LC26593) and the founder of the Care Map Method™. I’ve worked directly with families for more than 20 years. My book, Secrets of the Nanny Whisperer (Penguin), is about finding and working with a nanny; my therapy work draws on my broader clinical training.

I’ve also shared parenting and family guidance as an expert for outlets including the Today Show, Good Morning America and The New York Times. Press and media.

Questions people ask

Is it normal to feel relieved to go back?

Yes. Many parents feel relief and guilt, sometimes on the same morning. Both are normal.

Can therapy help me choose childcare?

We’ll work on what’s making the decision hard. For hands-on help choosing and hiring a nanny or daycare, Care Map coaching is a separate service that isn’t billed to insurance.

What if I want to stay home instead?

Then we work on that decision: what you want, what’s possible, and how to talk with your partner about it.

When should I start?

A few weeks before your return date gives you time to plan, but any time is fine, including after you’re back.

Do you take my insurance?

I see clients through Alma, and through Alma I’m in network with Aetna, Cigna, Carelon, Blue Cross Blue Shield of Massachusetts, and Optum plans including UnitedHealthcare, Oscar and Oxford. Coverage still depends on your specific plan. Alma checks it before your first appointment, including any copay or deductible. Please share plan and health details through Alma’s secure intake, not by text or email.

Where do I need to be during sessions?

Sessions are online by video, and you need to be physically in California, New Jersey, Oregon or Maine during each one. Your insurance plan can come from another state; what matters for licensing is where you are during the session. Alma checks whether your plan covers sessions with me.
